As part of our service we have set up a system to record our clients’ long-term care policies.

For this, your assistance is needed.

Please return the enclosed sheet so that we can make sure that everyone who wants this coverage has a policy – or has access to a quote for a long-term care policy. If you currently own a policy, please complete the form so that this is included in any future planning process. 

If you do not have long-term care insurance and do not wish to review long-term care insurance options, we would need your indication for our records.
